The Organization

JPS Heath Network is Tarrant County’s $910 million public healthcare provider, dedicated to improving healthcare delivery and quality of life for all county residents.

The network’s flagship is John Peter Smith Hospital in Fort Worth, a 578-bed acute-care hospital south of downtown. It is home to Tarrant County’s only Level I trauma center, providing state-of-the-art care for life-threatening injuries and coordinated medical response capability for mass-casualty events in the community. JPS is also a Comprehensive Level I Stroke Center and is AMI certified by The Joint Commission, providing the best possible care for heart attack and stroke, the leading causes of death and disability in the U.S.

JPS is also home to Tarrant County’s only psychiatric emergency center, meeting a critical need for families facing mental health crises; and Trinity Springs Pavilion, an inpatient psychiatric hospital for adults and adolescents. JPS locations also include the full-service center for women and children, and outpatient clinics dedicated to all major medical specialties. JPS Health Network also operates 11 nationally recognized medical home clinics throughout Tarrant County, strategically located to meet the needs of the communities it serves.

JPS is a teaching hospital, dedicated to training tomorrow’s physicians and other providers. JPS sponsors 17 residency programs and two fellowships, including the nation’s largest family-medicine residency, and is a clinical training partner in three additional residencies sponsored by Baylor and UT Southwestern.

The Opportunity

The Executive Director Clinical Integration and Care Transitions assesses, develops and implements, in conjunction with network physician, nursing, quality and financial leadership, continuum of care services and utilization management in order to deliver superior clinical and operational alignment across the Network.  The Executive Director directly oversees inpatient case management, outpatient case management, utilization review and social work services within the Network.  Additionally, the Executive Director Clinical Integration and Care Transitions is a strategic leader in programs supporting the transition from a fee-for-service, volume-based care delivery system to a value-based care model.  Success for the Executive Director Clinical Integration and Care Transitions will be measured and defined by the degree in which the Network is able to implement safe, appropriate, effective and efficient a) utilization management processes; b) care coordination activities; c) population health programs across outpatient-inpatient-post acute care settings; d) total cost of care initiatives; e) linkage for high risk patients; and f) accountable care type opportunities.  This position requires close collaboration across multiple disciplines with both internal and external partners, and must align tightly with outpatient, emergency department and inpatient nursing/operational teams to promote a safe environment of care and facilitation of effective discharge planning.

The Candidate

The successful candidate MUST have:

  • Master’s degree in Nursing, Business Administration, Healthcare Administration or related field of study from an accredited college or university.
  • Seven plus years of experience in Quality, Care Management or Care Transitions.
  • Current licensure by the Board of Nurse Examiners for the state or proof of reciprocity of licensure between the state and another state

Required Licensure/Certification/Specialized Training:

  • Current licensure by the Board of Nurse Examiners for the state or proof of reciprocity of licensure between the state and another state.
  • Experience using Microsoft Professional Office Suite, including Word, Excel Outlook and PowerPoint.
